I hope this post finds you well. If you’re looking to confirm the piriformis as a cause of sciatic nerve compression, the FAIR Test is for you. With your patient in a side-lying position, simply (F)lex-(A)dduct-(I)nternally (R)otate the affected lower extremity to the position pictured below. Sensitivity of 88% and Specificity of 83% makes this test a keeper for me. I suggest the limb be flexed no higher than 60 degrees. This will help maintain the piriformis’ function as external rotator and subsequently stretch it during internal rotation.

Another great tip Mark! I appreciate the reminder to not flex more than 60 degrees. I will be using this test later this week. I have the perfect subject!
